Lloyds of London is an "A" rated superior security by A.M. Best, financial size XV (excess of two billion dollars) and an S & P rating of excellence in claims servicing.

Egg Donor Recipient Complication InsuranceSM Summary Of Benefits

Plan Maximum:	$500,000 combined single limit for egg donor and recipient.
Coverage:	Complications only.
Accidental Paralysis:	$200,000 Combined single limit for egg donor and recipient.
Accidental Death Benefit:	$200,000 Combined single limit for egg donor and recipient.
Coordination of benefits:	Policy is secondary.

Medical Plan Premium And Deductible

Premium	US $395.00 Due per cycle.
Deductible	$0
Coverage Term:	The policy remains in effect (4) months from the reported cycle start date provided that complications are incurred within ninety(90) days of the completion of the cycle.

Preexisting Limitations And Exclusions

Exclusion:
Any claims reported outside of the US. Any complication resulting in claims not directly related to the egg donation and retrieval process.

Requirements:
The Egg Donor Recipient Complication InsuranceSM must be purchased per cycle.

It is a condition and warrantee that the egg donors and recipients are in good health, medically examined, interviewed and cleared by a three year or more board certified reproductive endocrinologist.

Limitations:
This Certificate provides limited benefits, and should not be considered Basic Hospital or Basic Major Medical Coverage. Benefits under this Certificate will be secondary, in excess of any other collectable medical expense insurance.

Pre-existing Conditions:
This policy covers no Pre-existing Conditions.
