Cost and Terms

Enrollment Fee Your Surrogate may apply for any of our exclusive insurance plans as a "Back Up Plan" by paying an enrollment fee of only $3000 to secure an insurance approval through underwriting. The enrollment fee is non-refundable. If insurance is activated enrollment applies to premium. Underwriting terms and conditions may apply.
Premium and Deductible Zero Premium or Deductible is due unless you activate your surrogate maternity insurance.
Activation is as
Easy As 1, 2, 3
1. Email New Life when ready to activate your Back Up Plan 3000.
2. Fund the full premium and deductible per plan purchased.
3. New Life Agency will retrieve a medical confirmation of
pregnancy form and issue the Surrogate Maternity
Coverage Certificate.
Claim Payments Denial of Major Medical claims submitted for processing on New Life's surrogate maternity insurance policies must be stamped received within 6 months from the date of service.

Claims Management and Customer Care

New Life Agency Claims Division is a third party administrator providing customer care and claims handling services, exclusively to Intended Parents and their Surrogates for New Life Agency, Inc.
